Honeywell Home Monitors

Health Related Home Care is excited to utilize the latest in medical devices in the care management of our patients afflicted with Congestive Heart Failure. As many will tell you, this disease can necessitate frequent visits to the emergency room and stays in the hospital. According to the Agency for Healthcare Policy and Research, 45% of all CHF patients return to the hospital within 90 days of initial treatment. Not only are these visits expensive, they are emotionally distressing and disruptive to the lives of our patients and their families.

The automated system greets the user each day and guides them through a simple three minute step-by-step process to collect their health information including: heart rate, blood pressure, weight and oxygen saturation. The user is also asked simple yes/no questions to further assist clinicians in evaluating their health. All of this information is sent directly to HRHC via telephone line, for review by their nurse. With the Honeywell monitor in place, our staff will be able to use the information it collects daily from the patient to better manage their plann of care and avoid frequent trips to the ER.
